Nursing Home - 90 year old resident fell and complained of hip pain. Relying on a negative x-ray report, the NH failed to obtain further medical assistance in spite of her ongoing complaints of severe pain for several days. It was later determined that the victim had suffered a hip fracture in the fall.

Assisted Living Facility - 91 year old resident of "dementia unit", a known wanderer, climbed out of an unsecured upper floor window and sustained fatal injuries.

Nursing Home - Upon entering mother's room, family members observed her in bed complaining of great leg pain, and with her leg rotated. X-rays confirmed a traumatic hip fracture, most likely from a fall. NH staff could not explain how a fall occurred, how she got back into bed with such a severe injury, or why medical treatment was not rendered earlier.

Nursing Home - After a short period of nursing home rehabilitation, an elderly dementia patient was discharged home from the facility with undiagnosed advanced gangrene in his foot, resulting in amputations and death.

Nursing Home - Newly admitted resident was erroneously administered numerous medications ordered for another resident, causing significant injury.

Nursing Home - Resident, who was at high risk for the development of pressure ulcers, was required to be provided pressure reducing devices. The devices were never provided as ordered, even though the NH's medical and other documents stated that they were. The victim, as a result, developed severe pressure ulcers causing death.

Nursing Home - Victim died when a large piece of hardened food became lodged in her trachea. She was not supposed to be served hard food due to her high risk for choking.

Assisted Living Facility - Resident sustained a hip fracture when she was allegedly struck by a wheelchair being pushed by a staff member.

Nursing Home - Resident with dementia, sustained fractures when, in an attempt to get out of her bed, she climbed over unauthorized bed side rails and fell to the floor.
